On May 1, 2004, Nuuanu Memorial Park took over operations of the mortuary facilities. The East Chapel was renovated and is air conditioned with state of the art audio visual equipment. The Chapel of the Flowers is currently being renovated with minimal changes in order to maintain its original architecture. Both chapels are equipped to service the various religious denominations. The East Chapel is adorned with a beautiful stained glass feature. Both chapels have authentic Buddhist shrines for Buddhist services. METHOD OF PAYMENT
Payment for goods and services is expected before the services are rendered, and, as such, are subject to the terms and conditions as set forth in the Statement of Funeral Goods and Services and the General Authorization / Agreement for Funeral (Burial or Cremation) Arrangementsdocuments, unless other arrangements have been made prior to need. All charges are subject to current State of Hawaii General Excise taxes where applicable.
For your convenience we accept cash, first-party checks (personal, certified or cashiers), credit card (Visa, MasterCard, Diners Club International, and JCB Cards-except for cash advanced charges), a verifiable irrevocable insurance assignment, or financing through a financial institution. Our mortuary does not have "in house" financing or a monthly payment plan, nor does it accept or credit to the account any unverifiable assignments of funds by any governmental, public or private entity.
SERVICE PAYMENT INFORMATION
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

Basic Professional Services of Funeral Director and Staff |

| This is the basic fee that is charged to every family we serve for our professional expertise in assisting them. It includes, but is not limited to, a proportionate share of the taxes, licenses, utilities and business expenses necessary to serve the public in a professional manner. Also included in this charge is the consultation with the family, clerical and administrative services, preparation and filling of necessary permits, consultation with the clergy, cemetery, crematory or common carrier, planning the funeral, and placement of obituary notices. There will be an additional charge for the direction and supervision of each service requested and required. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. Embalming |

| Embalming is a chemical process which provides temporary preservation of the body and eliminates certain health hazards. A person licensed by this state must complete this procedure. Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you have the right to choose an arrangement that does not require you to pay for embalming such as direct cremation or immediate burial. |

We offer many different styles and prices of caskets, and alternative containers from which to select. Since many caskets that appear similar in appearance may differ greatly in quality and construction, we offer the following in order to assist you in making an informed decision. |

Protective: These caskets are designed by the manufacturer to resist the entrance of air, water and other outside elements. They may be constructed of varying gauges of steel, cooper or bronze. There is no scientific or other evidence that any casket with a sealing device will preserve human remains. |

Non-Protective: These caskets are not designed by the manufacturer to resist the entrance of air, water or other outside elements. They may be constructed of metal, hardwood or wood products covered with fabric. A complete list of the caskets offered will be provided with this general price list. |
